I didn't have such a bad misfire as dude, but had all the same codes. For me it ended up being a vacuum leak. You can try spraying brake cleaner to locate the leak, but that didn't work for me. I took the lower intake off and was able to see the severed line then.

Give it a good look around. I think that you can get a vacuum leak from the SAP system (pump/hoses), intake after the MAF, or hoses under the intake.
