Default File uploads failing

I'm trying to upload an ETM (in PDF format) to the "downloads" section, but it's not working.

I've filled out the form...

File Name:

In the "Add File" section...
... browse for file... select file... click "Add File"... then a pause while the file would theoretically be uploading. But, I get an error messsage:

"Fields marked in RED must be filled in!"
But, no fields are marked in red. Everything is still filled in with the exception of the Description and the file down in the "browse" field.

Am I simply doing something wrong, or is there an issue that needs to be resolved?
